additional field in checkout module ??? how to insert?

I need to insert an optional text field, what can be filled out by customers.
that field need to be under every picture in the shopping cart.
How can i do that?

if this will not be possible so i need to create one big text-box into the checkout process,
where the customer can write special wishes concerning the photos he buy.
