Need a quote on some changes please

I need a quote on changing a few simple things on my current gallery (http://glutenfreepost.com/Recipes/main.php)…
1. The Gallery logo must be changed to my logo
2. My new logo must also be a link back to the main gallery page
3. Create sidebar links that do the following:
• Take the user back to the main Blog (www.glutenfreepost.com)
• Take the user to the Blog Forum (http://glutenfreepost.com/forum/index.php)
• Create a link where the user can upload a picture, 640x480, along with a description of the recipe, that pic must be approved by the admin prior to posting
4. And lastly, create sidebar space where html ads and Google ads can be placed. And include instructions on how to place those ads.
