I was hoping to get this add-on, but it appears the link is invalid:

Add captions when uploading (Kommercial)
Allows user to attach Name/Email/Web addy/Caption to picture WHEN they upload it
public classifieds also possible using this

You should upgrade to Gallery v1.3.2!
Add captions when uploading was added:

But his also includes email, etc.

You might want to setup that layout with tables -- it would look a lot better. Cool idea, though.

Sorry, deleted it off the server with a bunch of stuff without realizing. It should be working now.
