what are the steps to reproduce the error? modifycartcontroller in the stack trace makes it look like this was not a photoaccess operation. it looks like an item in your cart was deleted from the gallery?
also note that lib/tools/phpinfo.php requires your config password, so posting it here doesn't give us any info.. (but thanks for all the other details!)

Hmm, this error might still have something to do with being logged out automatically. I have been trying to change sections in the Admin area, and get a permission denied error and click back on gallery only to find that I am no longer logged in.

I think I also read that photoaccess was bought by someone else recently, so them breaking that may be another option. Maybe have a test with G1 and see if everything still works there?

Photoaccess is still working, I just tested it now. Let's see if we can figure out your "keep getting logged out" problem (in your other thread) and see if resolving that makes this problem go away.
