Is there a way to upload a different image to the second pho

What I am trying to do is when you click on the thumb I needed the second photo that comes up to have orig photo (thumb) with a cad photo next to it with a small graph I can design the entire photo in photoshop. What I wanted to do is either upload it logging in via ftp (which I tried to upload the file) but it won't let me overwrite the file (I am naming the second pic the same name as the orig second pic). Or preferably I would prefer to upload the image using the gallery. Right now you are able to edit the thumb which changes the entire photos (thumb, medium, large). I would like to change the medium and leave the thumb and large as the same photo.

please note the fireplace is the thumb and large picture -- the above photo will only be the second picture after thumb

Gallery is designed as a photo gallery that shows the same picture as a thumbnail and (if needed) resized version. It is based on the original picture you upload. In this case, you want the resized version to be different than the original. Gallery is not designed for this and hence doesn't support it.

However, there is one way to make gallery do what you want. (Actually, there are two ways, but the second requires a lot of coding, I'm assuming you don't want to do that.)
You should use Bharat's <!-- BBCode Start --><A HREF="http://gallery.menalto.com/modules.php?op=modload&amp;name=GalleryDocs&amp;file=index&amp;req=viewarticle&amp;artid=22&amp;artid=22" TARGET="_blank">Cleanup script</A><!-- BBCode End --> to have permissions to overwrite the resized files. Then you replace the resized files manually with your own. But keep in mind that the width and height should be the same, otherwise they won't display correctly.
