I have two galleries one stopped working and I get this error when I try to open it.

I apologize for not having the exact information on the bottom of the form and the mistake on the version. When I clicked on the information for my other gallery that version came up.
The gallery has been up and functioning properly for several months.
I reloaded the both of the index.php files. The errors did not change.

What did the file integrity say?
Seems to be a host issue as:

I am confused are you saying the php info link I listed is not working or that I put the file in the wrong location?

For the 3rd time, what did the file integrity say?
That is G1. If you want to test install another G2 and see if it works.

These are the instructions for getting file integrity, is it not?
"Browse to http://www.example.com/gallery/upgrade/index.php or http://www.example.com/gallery/install/index.php and advance to the System checks step. Make sure there are no warnings, and especially that the file integrity check passes (no missing / modified files). If there are missing / modified files, read this FAQ entry to fix it."
Is there a way to get the integrity file without using one of these indexes?

Are you sure that is right? because previously ( first post ) you said Gallery v1
If not then lets verify with vsersion.dat in the g2data directory.
Then lets over write the files you have with a fresh set of files from that version. here is 2.1.2:

verify a few files by looking at the top of the file for the @version $Revision: ####### $ to see if you have the right version.
