[SOLVED] unable to add photos after upgrade to 2.1.1 - mysql duplicate entry - ERROR_STORAGE_FAILURE

After upgrading to 2.1.1, I am no longer able to add photos. Occasionally I'll see an imageless addition (Title and Description but no image). Near the bottom of the debug info I see:

file_exists(...GalleryStorage_describeMap.inc) mysql error: [1062: Duplicate entry '18' for key 1] in EXECUTE("INSERT INTO g2_AccessSubscriberMap (g_itemId, g_accessListId) VALUES (18,15495)")

but I'm not sure what to do about that. Can someone point me in the right direction?
