[SOLVED] G3 folder structure corruption??

Been having some big problems with uploading photos recently - my folder structure seems to be corrupt.
Most recently reproduced error (I left a buggy picture in there for you to see):
I have a folder in Gallery called Maps. Maps has four subfolders: "120x120", "100x150", "120x240" and "cases".
I just tried uploading a photo into the 100x150 subfolder, and it uploads fine and shows up in the folder, but with a broken thumbnail.
The photo should be in gallery/maps/100x150/, but when you hover over the thumb, the system thinks it should be in gallery/maps/100x150/120x120 (which does not exist).

This happens pretty much every time I upload now (both regular upload and server add), and I have no idea where the corruption started, lives, or how to fix it.
When looking at the FTP server, I can't seem to find the uploaded image, and the erroneous subfolder (100x150/120x120/) has not been created.
Any suggestions? =(
Running Gallery 3.0.1 with greydragon.
